Visual rehabilitation after failed therapeutic PK with complicated cataract is challenging because of poor visibility and altered anterior chamber dynamics. Manual small incision cataract surgery along with DMEK is possible - an extended indication. This is a story of one-eyed farmer who had mixed corneal ulcer with perforation. We managed him by urgent Therapeutic PK followed by DMEK with IOL.
