A 14 year old male presented with a traumatic white cataract and 5mm fibrotic anterior capsular plaque. We demonstrate cataract extraction and intraocular lens placement, using intraocular scissors and capsulorhexis forceps to create a capsulorhexis around the plaque, followed by vitrector aspiration of lens material and insertion of a 3-piece IOL. Plaque histopathology showed predominantly heme.
